RELEASE OF LIABILITY FOR MINOR PARTICIPANTS

IN CONSIDERATION OF my registered child/ward being allowed to participate in any way in The Marblehead Girls Basketball League Corp. related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
2. I, for myself, spouse, and child/ward, knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my child/ward’s participation.
3. I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/ward’s readiness for participation and/or in the program itself, I will remove my child/ward from participation and bring it to the attention of the nearest official immediately.
4. For myself, my spouse, my child/ward, and on behalf of our heirs, assigns, personal representatives and next of kin, I release and hold harmless The Marblehead Girls Basketball League Corp., its directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property incident to my child/ward’s involvement or participation, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.
5. For myself, my spouse, my child/ward, and on behalf of our heirs, assigns, personal representatives and next of kin, I indemnify and hold harmless all the above Releasees from any and all liabilities incident to my child/ward’s involvement or participation, even if arising from their negligence, to the fullest extent permitted by law.

By selecting Yes, you acknowledge that you have read this Minor Waiver/Release, fully understand its terms, understand that you have given up substantial rights by signing it, and sign it freely and voluntarily without inducement. You understand the seriousness of the risks, your responsibilities for following rules and regulations, and accept them for the participant.

AMATEUR ATHLETIC WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in The Marblehead Girls Basketball League Corp. athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the Releasees or others, and assume full responsibility for my participation.
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring it to the attention of the nearest official immediately.
4. For myself and on behalf of my heirs, assigns, personal representatives and next of kin, I release and hold harmless The Marblehead Girls Basketball League Corp., its directors, officers, officials, agents, volunteers and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event (“Releasees”), with respect to any and all injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

For participants under age 18: As parent/guardian with legal responsibility for this participant, I consent and agree to their release as provided above. For myself, my heirs, assigns, and next of kin, I release, indemnify, and hold harmless the Releasees from liabilities incident to my minor child’s involvement or participation, even if arising from their negligence, to the fullest extent permitted by law.

By selecting Yes, you acknowledge that you have read this Adult Volunteer Waiver/Release, fully understand its terms, understand that you have given up substantial rights by signing it, and sign it freely and voluntarily without inducement.

The following constitutes the policies of Marblehead Girls Basketball League Corp. regarding awareness and prevention of abuse within our organization.

Marblehead Girls Basketball League Corp. is committed to providing a safe environment and preventing child abuse and sexual misconduct. The league will make every reasonable effort to ensure every person involved in coaching or training abides by the SafeKids guidelines, exclude any adult with a legally documented history that would bring unnecessary risk to participants, perform criminal background checks for every person applying for a position, and take appropriate action on all allegations. Allegations will be reported immediately to authorities, and the league will cooperate fully with investigations.

• Physical, mental, and verbal abuse is not permitted.
• Inappropriate touching is forbidden.
• More than one adult will oversee activities and individual attention.
• Coaches and trainers should not socialize with participants outside sponsored activities or ride alone with a child.
• Parents are encouraged to attend sponsored activities.
• Allegations will be reported immediately to authorities, and MGBL will cooperate fully.
